Change of major, minor, or advisor

Form for students who wish to change their major/minor/advisor

Change of name or address

This is the form to change your address or name in the Saint Martin's University database.

Complete withdrawal from Saint Martin's University

This is the form to completely withdraw from Saint Martin's University.

Course add/drop form

(Only for use if Self-Service is unavailable)

Use this form to add or drop courses from your schedule. You may indicate if you are dropping all courses for the semester and/or requesting Voluntary Medical Leave. This form must be signed by your advisor and returned to the registrar's office. If you are completely withdrawing from Saint Martin's, please complete the Complete Withdrawal from SMU form.

Course substitution and waiver form

Use this form to substitute a course or waive a course.

Direct study request

Form for a student to propose a course of study.

Enrollment verification

This is the form used to verify enrollment through signatures from your professor.

General Education Course Substitution and Waiver Request Form

This form is for students wishing to substitute or waive a gen ed.
