Referral Information

When Do I Make a Referral?
Consider making a referral if, after speaking to a student about his/her academic standing and/or concern, no improvement occurs. A convenient way to refer a student for assistance is by utilizing the centralized SSI referral system. This Early Identification and Referral (EIR) form is designed to facilitate communication between the referring faculty/staff member and the Learning Center.

Examples might include:

  • Student A is repeatedly late to class/high absenteeism
  • Student B performs poorly on exams because they don't keep up with the course readings and lectures
  • Student C appears to be under acute stress due to personal matters and is beginning to fall behind in coursework

How Do I Make a Referral?
To refer a student using the online EIR form (click here for online EIR form). All EIR forms are sent to a dedicated email account in the Learning Center. These referral forms are used to initiate a contact with the student. Keep in mind, the information you write on the form may be shared with the student.

Behavioral Intervention Team: If you need immediate help due to a student's threatening behavior that could pose potential harm to self or others, contact the Dean of Student Services (Old Main 206, 438-4367)

What happens after the EIR form is submitted?

Once an EIR form has been received by the Learning Center, an EIR response message will be emailed to the student inviting him/her to arrange an appointment with the Learning Center Director. A copy of the EIR response will be sent to the student’s advisor and the referral source (click here for sample EIR response). The intervention may vary depending on the circumstances reported.

Possible interventions:

  • Student A accepts the invitation to attend an individual learning consultation in the Learning Center in order to address time management challenges.
  • Student B discloses that they are dyslexic and are referred Disability Support Services for further evaluation.
  • Student C is referred to counseling services. Student is also considered for one of the weekly Student Success Committee meetings in response to multiple EIR forms received that express concerns about this student.

The Student Success Committee is comprised of the Dean of Students, Dean of Admissions and Enrollment, Learning Center Director, Disability Support Services Director, Intercultural Initiatives Director,  Residence Life Director, and First Year Seminar Director. Academic advisors are always notified and invited to attend these meetings when one of their advisees is scheduled for discussion. Other university personnel may be asked to attend when appropriate. The purpose of these meetings is to collect relevant information from key departments on campus so that a course of action can be developed to best address the needs of the student. In accordance with the Family Educational Rights and Privacy Act (FERPA), information will be shared on a need-to-know basis and will not become part of the student's permanent record FERPA guidelines
