Student health insurance
Students who are enrolled half-time or more on the Lacey campus are
required to have health insurance coverage. Students who have their own
coverage must submit an online waiver form directly to the University's
health insurance provider by the first day of the semester. Waivers are
good for the academic year provided that the coverage listed has not
changed. It is the student’s responsibility to notify the Student
Financial Service Center in the event of any changes. Those who do not
have their own personal coverage, are required to purchase insurance
through the University’s provider.
All international students are required to have health insurance
coverage. The claim form and online waiver link listed below do not
pertain to international student insurance. Those students must contact
the Office of International Programs 360-438-4375 for more information.
Download and print the
international waiver form here. The completed form must be received
in the Office of International Programs by the first day of the semester
to be valid for that enrollment period.
To waive the domestic student health insurance, complete and submit the
online waiver form by the first day of the semester. You
must use your complete SMU ID# which includes the leading 'P'
when entering your number on the waiver. You will receive an 'approved'
or 'denied' response. If your waiver is accepted, the insurance charge
will be removed from your account within approximately three business
days. If your waiver is denied, you will have the opportunity to appeal
the request directly online. Approved appeals will allow SMU to remove
the insurance charge. Denied or no appeals will result in the
student being responsible for the insurance charge. Students
who have not submitted a waiver form by the first day of the semester
will also be charged the insurance premium fee and will not be eligible
to have the fee waived later in the semester. Be sure to have your
waiver form submitted and approved by the deadline. Waivers cannot be
accepted after this date.
Students needing the University's insurance plan need not do anything.
You will automatically be enrolled in the plan if no waiver is received.

Those students who are receiving insurance coverage through the
University, must submit a claim form to their physician at the time of
service.
Aetna Student Health claim forms are available in the Student Financial
Service Center or can be
downloaded and printed
from this site.
Aetna preferred provider physicians offer better coverage, however, you
may choose any doctor you wish. For a list of preferred providers, go to
