Intake form
Students new to DSS are encouraged to fill out an
Intake form. The
information in the form helps the DSS office in the provision of
accommodations. This packet includes gathering student information,
a release of information document, test taking guidelines, and
student rights and responsibilities. Please download this packet,
fill it out, and return it to Geoffrey Brown, DSS coordinator, in
the DSS office. The DSS office is located in the lower level of
O'Grady Library.
